Here are the steps to get ISIS to show you what should be the correct values for UpperLeftCornerX and UpperLeftCornerY.
pds2isis from=ldem_4.lbl to=ldem_4.cub
these values are bad:
UpperLeftCornerX = -5450623.96
UpperLeftCornerY = 2721521.56
2.) To have ISIS calculate the correct values we can run maplab
maplab from=ldem_4.cub map=ldem_4.cub sample=0.5 line=0.5 coordinates=latlon lat=90 lon=0
these are the correct values (or very close to) that pds2isis should be using in the label:
UpperLeftCornerX = -5458203.0763469
UpperLeftCornerY = 2729101.5381735

we need a deployment plan for #2244 before this can be committed. Code in http://subversion.wr.usgs.gov/repos/prog/isis3/branches/m02244/isis
test data is already checked in to Isis3 data directory. See #4507 for other notes on deployment.

To deploy this without breaking old versions of ISIS I suggest we change the code to use a different file name for the specific (non-default) offsets. This way old version of ISIS will continue to use the original file, and new version of ISIS will only look at the new file. NOTE: Both files should be documented well to show exactly which versions of ISIS they are expected to work with (including beta and production versions)
